Tips clerk psychiatric patient

Bosan betul kalau exam lambat lagi ni. Dah xtau nak buat ape, selain dari study (wah, innocent bunyinye~). Malang sekali... Sekarang ni tengah belek2 buku psikiatri, kononnye nak baca sikit2, panas enjin dulu. Tapi entah ape yang dah baca agaknye... Memandangkan Fana asyik buat entry ilmiah pasal paeds n ortho sempena exam ni, aku pun nk buat juga. Aku nak kongsi satu tips dari Dr. Tuti. Bukan soalan bocor ke ape, tapi panduan pasal macam mana nak clerk pesakit psikiatri dalam masa yang singkat.

HOPI:
1) Background history of pychiatric illness (with medication)
2) Current psychopathology (don't list d chronology, except the patient have co-morbid illness)
3) Exclude relevant negative (e.g: mood disorder, substance induced, medical illness)
4) Any treatment before???
5) Functioning prior to this visit/admission
6) Current stressor (e.g: compliance, HEE)

Past psychiatric history:
1) Last diagnosis (sometimes berubah)
2) Previous treatment
- medication (compliance, side effects, response)
- psychotherapy
3) Admission (how long?)
4) Any relapse???
5) Longitudinal functioning (fungsi pesakit semasa sakit)

Management:
- manage patient as a whole, settle issues
- biopsychosocial
