Let's revise O&G procedures

Next week adalah minggu terakhir untuk posting O&G, dan juga minggu terakhir dalam final sem. Lepas tu dah masuk minggu apungan (bak kata Prof H) atau study weeks selama 4 minggu. Dan kemudian, say hello to professional exam!!! Takutnyer...

Just nak share procedures kat sini... Jom same2 revise OSCE O&G~

A) Urinary catheterization
1. Introduce and consent.
2. Expose the patient.
3. Position the patient into dorsal position.
4. Prepare the equipment (urinary catheter, syringe 10cc, water for injection, cotton, normal saline, urine bag).
5. Aseptic technique and wear sterile glove.
6. Clean and drape.
7. Identify the external meatus.
8. Insert catheter until urine come out. Insert 3-4 cm more.
9. Inflate the balloon.
10. Pull catheter gently until feel resistance.
11. Connect the catheter to urine bag.
12. Reposition the patient.

B) Pap smear
1. Introduce and consent.
2. Prepare the equipment (Cusco speculum, Auer's spatula, lubricant, slide, 90% alcohol spray, light source).
3. Position the patient into dorsal position.
4. Clean the area.
5. Insert speculum.
6. Take sample with spatula at endocervix, ectocervix and posterior fornix.
7. Smear the sample on slide.
8. Spray slide with alcohol.
9. Remove the speculum.
10. Reposition the patient.

C) Dilatation and curretage
1. Introduce and consent.
2. Put patient under anaesthesia.
3. Position the patient into lithotomy position.
4. Clean and drape.
5. Catheterization.
6. Do bimanual examination to check the size and position of uterus, cervical os, adnexa and POD.
7. Insert Sim speculum.
8. Grab the anterior lip with Volcellum forcep.
9. Dilate the os with Hegar dilator.
10. Introduce uterine sound.
11. Currete until feel gritty sensation or presence of whittish tissue.
12. Check for any bleeding.
13. Remove the speculum.

D) Marsupialization
1. Introduce and consent.
2. Put patient under anaesthesia.
3. Position the patient into lithotomy position.
4. Clean and drape.
5. Catheterization.
6. Identify the swelling.
7. Identify the mucocutaneous junction.
8. Make cross (X) shaped incision.
9. Drain the fluid.
10. Flush with normal saline or hydrogen peroxide to clean.
11. Suture edges to skin.
12. Pack with ribbon gauze soaked with povidone.
13. Wear sanitary pad.
14. Remove ribbon gauze the next morning.
15. Dreassing and leave open until heal.
